These terms apply to services provided by Cr Medi Consult LLC, doing business as Stat Labs ("Stat Labs", "we", "us"), a mobile phlebotomy and paramedical exam service in New Jersey.

Services

Stat Labs performs paramedical exams for insurance applications and private blood draws ordered by a patient's doctor. Scheduling is arranged directly with you.

 

SMS Terms

You agree to receive appointment texts from Stat Labs when you reply YES to our first text message, or tell us by phone or in person that text messages are okay.

- Program: Stat Labs sends appointment-related texts only: scheduling, confirmations, reminders and changes. We do not send marketing messages.

- Message frequency varies, depending on your appointment.

- Message and data rates may apply.

- To opt out at any time, reply STOP. To get help, reply HELP or contact us at office@statlabsnj.com or [347-684-3120].

- Carriers are not liable for delayed or undelivered messages.
