Cr Medi Consult LLC, doing business as Stat Labs ("Stat Labs", "we", "us"), is a mobile phlebotomy and paramedical exam service in New Jersey. This policy explains what information we collect and how we use it.

Information we collect

When you request or are referred for a paramedical exam or a private blood draw, we receive your name, phone number, address, date of birth, and details about the requested exam or tests. We receive this from you, your insurance agent or insurance company, or your doctor's office.

 

How we use it

We use your information to schedule and perform your appointment, to contact you about it (by phone, text message or email), and to deliver the specimens or results to the party that requested them, such as your insurance company, its laboratory or your doctor.

 

Text messages

If you give us your mobile number, our first text asks you to reply YES to agree to texts. After you agree, we may text you about your appointment: scheduling, confirmations, reminders and changes. We do not send marketing messages. You can reply STOP at any time to stop receiving texts.

 

We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes.

 

Who we share information with

We share information only as needed to carry out your appointment, for example with the insurance company or laboratory that requested the exam, or with your doctor, and as required by law. Text messages are sent through our messaging provider, which handles them only to deliver them to you.

 

How we protect it

We limit access to our records and protect them with a password-protected system. No method of storage or transmission is completely secure.

 

Your choices
